To book, ring the practice and say: Ich hätte gern einen Termin. — I'd like an appointment. (Einen, as in einen Kaffee: der Termin is masculine.)

Like most job titles, Arzt adds -in for a woman — with an umlaut here: Sie ist Ärztin. No article before a job: Er ist Arzt. Traditionally, patients say Frau Doktor or Herr Doktor; many simply use the surname: Guten Tag, Frau Schneider.
